What to Expect on the Consent Form

The online form will collect the same information as the paper version — securely and conveniently.

Student Information

Basic details about your child: name, date of birth, grade, school, and contact information for the parent or guardian.

A brief health questionnaire to determine vaccine eligibility, including allergy screening and any relevant medical conditions.

Parent Authorization

Your electronic signature authorizing Control Flu to administer a free influenza vaccine to your child at school.
